How to Enroll

To enroll your student please follow all instructions below

 

Returning Students

Download and complete the following items:

1. The Enrollment Packet

2. The Meal Application

3. The Charter School Enrollment Questionnaire.

Download the packets to your computer. Open the documents in Word and type in the information. Be sure to fill out each page COMPLETELY. We will need you to fill out any missing information to complete the enrollment process for your student. Once you have completed the packets, save the completed version to your computer, and email them to amartinez@por-vida.org.

We will contact you should we need any additional documents.


 

New Students

Download and complete the following items:

3. The Charter School Enrollment Questionnaire

In addition, we will need the following items:

1. The previous school's transcript

2. A copy of a government issued ID

3. Shot records

4. Social security card

5. Proof of residency. 

Download the packets to your computer. Open the documents in Word and type in the information. Be sure to fill out each page COMPLETELY. We will ask you to fill out any missing information to enroll your student. Once you have completed the packets, save the completed version to your computer, and email them to amartinez@por-vida.org.

 

 

All Students

If you have issues with access to a laptop or internet, all documents can be picked up outside the front doors of our school. Once you complete them by hand, you may drop them off in our secure drop box located outside the front doors of our school.

If you are having issues completing the packet, you may also click the button below to "Schedule an Appointment." If you opt to come to the school, your packet must be completed to the best of your ability before your arrival and you must be wearing a mask. Only one person will be allowed in the building at once, and you MUST have an appointment.

Por Vida Academy serves students in grades 9-12 who are looking for an opportunity to graduate from high school in a small, supportive environment.
