Eligibility Requirements

• Students must be at least 13 years old and have completed the 8th Grade.
• A parent or legal guardian MUST accompany all students under 18 years of age to enroll.

• Birth Certificate (State registered, not hospital issued) Request Birth Certificate
• Social security card (no copies please)
• Immunization Records (records must be up to date)
• Proof of Address(Utiility bill, etc.)
• Proof of Income
• Parent/Guardians ID and SS Card
• Official transcript/working document (obtain from previous school)
• Special Ed. Records
• Withdrawal/Drop Slip from last school attended
